The Irregular Surface Compensator (ISC) - Setup Verification and Delivery
On the linac, the patient is positioned as usual. before each ISC field, a setup field with static MLC (displaying the complete irradiation area outline, the CIAO field) is used to verify the correct position of the field. The images of the setup fields can be retaken several times if necessary. When setup is acceptable, the ISC file is loaded.
Fig.1: Setup fields for the two tangential breast fields. The static MLC shape helps when setting up the patient. It is generated out of the dynamic sliding window sequence by taking the left leaf bank of the first DMLC segment and the right leaf bank of the last segment. This is done manually by editing a copy of the DMLC file with a text editor.
 
When the ISC field is treated, the leaves move over the field (sliding window). For verification purposes, an integrated image is acquired, which sums up the photon fluence of the whole delivery (see Offline Review).
